This is the Extended Media Thickness Sensor for the Contex, GTCO Calcomp and Vidar scanners. Manufacturer's part number is 6864a102R01.
It is used on scanner models HA68A, HB68A and HC68A. You can find the model number on the back of the scanner. Also, the model number is the first 5 characters in the serial number. See below for a list of model names:
